The following is a list of the items we recommend that you bring with you for your first visit:

• A current written list of all your medications

• Copy of your previous medical records or name/address/number of where they are located

• Records Release Form(s) from your previous provider(s)

• Copy of your previous glasses/contact lens prescriptions

• Your current pair(s) of glasses and/or contact lens boxes
